Case Management to support people with a Lasting Power of Attorney (LPA)

What is Case Management?

A collaborative process which: assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individual’s health, social care, educational and employment needs, using communication and available resources to promote quality cost effective outcomes. Brief examples of ways in which we have assisted:

• Periodic reviews of people in care homes, focusing on practical aspects and quality of life. This may also include attending care home reviews and liaising with GPs and other professionals.

• Carrying out specific assessments for therapies and/or adaptations and equipment. We have knowledge of literally thousands of pieces of equipment!

• Liaising with professionals involved in care; sourcing appropriately qualified and experienced professionals to work with clients e.g., neuro-physiotherapist.

• Arranging for specialist taxi service to trips out of a care home environment

• For those living in their own homes, ensuring that these are set up as optimally as possible in terms of equipment and care, to support the client and to minimise risks. Liaising with statutory services for provision of equipment.

• Supporting family members emotionally with the ups and downs of caring for a loved one with health issues, including assisting with transition into residential care, if applicable.

•Supporting family members with raising any concerns to the relevant authorities and agencies, where necessary.

•Ensuring that the relevant benefits have been applied for, and if necessary, assisting with completing applications. This also includes Blue Badge applications.
